Though she and Rhys were now married, she still went by her maiden name of Gwen Cooper.

Gwen did not take a married name and continues to be identified as Gwen Cooper on both Torchwood and Doctor Who, reportedly at the request of Myles.

Yes, I went through the episodes and found that she was called herself Cooper 2 times and only once did jack call her Mrs. Williams but it was just in jest

it appears that although she's now officially/legally by marrage Mrs Williams, she still uses the name Cooper for work.... a bit like some teachers or Doctors who get married but still use their madien names at work....

Both my sister and daughter are married and both of them still use their maiden names for everything. They never had them legally changed.
